Width is measured across the ball of your foot, the widest part. For men, D is the standard (medium) width, EE is wide, and EEE (also called 4E) is extra wide.
Each step up adds roughly 1/4 inch across the ball, which is what the width letters mean in plain terms. One caveat: brands don't all label widths the same way, so an EE from one maker isn't always an EE from another.
